Admission, Discharge and Transfer Criteria

Admission Criteria

To be eligible for services the individual must:

  • Indicate a desire to participate in supported employment services in order to find a competitive job in the community
  • Be 18 or over
  • Have a significant disability
  • Be able to self-administer any medications needed while working. LFSVA staff cannot administer or store participants’ medications
  • Be able to care for personal needs or have personal care assistance provided
  • Have the funding necessary for receiving services (DARS, Medicaid Waiver, private pay)
  • Be willing and able to abide by all policies, regulations and safety practices of the program environment
  • Not be a clear and continuing danger to self or others
  • Participate, along with family member(s) or a designated representative, as members of the Interdisciplinary Team and cooperate in the development and implementation of the individualized service plan

Discharge Criteria

The individual will be eligible for discharge if/when the individual*:

  • Fails to meet and/or maintain the admission/retention criteria
  • No longer wishes to engage in services
  • Does not show up for scheduled meetings and appointments without a valid reason
  • Is stable in employment and no longer wishes to receive long-term follow-along services

*Additional discharge criteria may be identified

Transfer Criteria

The individual will be transferred from Situational Assessment services to Job Development services when the individual:

  • Continues to indicate a desire to participate in Supported Employment Services
  • Has identified an appropriate job goal during Situational Assessment services
  • Has received an authorization from DARS to begin Job Development Services (when receiving DARS funding)

The individual will be transferred from Job Development services to Job Placement and Training services when the individual:

  • Has accepted a job offer
  • Has procured adequate transportation to and from work
  • Has developed goals for Job Placement and Training Services
  • Has received an authorization from DARS to begin Job Placement and Training services (when receiving DARS funding)

The individual will be transferred from Job Placement and Training Services to Follow-along services when the individual:

  • Continues to display desire/motivation to work and maintain the job
  • Demonstrates stability on the job
  • Understands, along with the employer, that the Employment Specialist will reduce involvement but will be available as needed to assist with issues that arise
  • Has developed goals for Follow-Along services
